Vitreoretinal surgery outcomes identified for complex retinal capillary hemangioblastoma in 2024

Recent studies evaluated the functional and anatomical outcomes after vitreoretinal surgery for patients with complex retinal capillary hemangioblastoma (RCH). This case included fifteen patients and sixteen eyes with tractional or combined retinal detachment that were managed with pars plana vitreoretinal surgery and tumour endorsection with or without feeder vessel ligation.

The mean age of the patients at the time of their vitreoretinal surgeries was thirty years of age. Most common tumour locations were inferotemporal in six eyes and temporal in six eye quadrants. Indications for surgery included fibrovascular proliferation in eight eyes, eudative retinal detachment and rhegmatogenous retinal detachment. Vitreous haemorrhage was seen in four eyes.

Tumour destruction was performed using laser or cryotherapy, some had a combination of both in nine of the eyes while ER was used in seven eyes. The feeder vessel was cauterized in sixty three percent and the feeder vessel was ligated in thirty seven percent. The anatomical success after the initial surgery was fifty percent in eight eyes, improving to eighty eight percent in fourteen of the eyes after undergoing a repeat surgery for recurring retinal detachment.

Visual acuity improved in forty four percent of the eyes and was stable in twenty five percent. Thirty one percent had worsening visual acuity with a mean follow up of twenty nine months. Comparing the ER group to the laser / cryotherapy group showed no difference in the retinal detachment rate.

Ray Man, the PA for Mahi Muqit, a leader vitreoretinal surgeon in London, said “Pars Plana Vitrectomy with or without tumour endorsection can be a safe and effective treatment for complex RCH with good tumour control and anatomical success. Outcomes relating to retinal detachment have been discussed in numerous reports over the years and the study that was carried out was done to understand these outcomes when using vitreoretinal surgery for indications other than retinal detachment in eyes with retinitis pigmentosa.”

The study that was carried out used clinical data from January 2013 to December 2021 with patients being treated with vitreoretinal surgery invention. The outcome assessed the changes in the best corrected visual acuity.
